Information for Professionals
Achieve Therapy has a strong commitment to building relationships with professionals who make referrals to us or who may work with us for other reasons. We promote open communication and information-sharing to achieve gold-standard services for our clients. We are also interested in learning more about how we can make referrals to others who can help the clients we work with.
Who can make a referral?
Referrals to Achieve Therapy from professionals are always welcome. We accept referrals from teachers, childcare staff, GPS, paediatricians, nursing home staff, and any other person who has a concern. Children can also be referred to Achieve Therapy directly by their parents or legal guardian. Adults can self-refer if they would like help from an Occupational Therapist or Speech Pathologist. Please be aware that we will only accept referrals of children where their parent or legal guardian has consented to the referral.
How do I make a referral?
Referrals can be made by phoning (02) 8739 7591 or emailing achieve@acheivetherapy.com.au. You can also complete an online referral form.
GPs, paediatricians, and psychiatrists can refer patients under mental health care plans (OT only), chronic disease management plans or Autism assessment and treatment plans for services that receive a Medicare rebate.
